UICC has launched the 9th edition of its TNM cancer staging system, Edited by Dr James Brierley, Dr Meredith Giuliani, Dr Brian O’Sullivan, Dr Brian Rous and Dr Liesbet Van Eycken. this new addition provides the latest, internationally agreed-upon standards to describe and categorise cancer stage, reflecting advances in imaging, tumour biology, and patient care.

With revised criteria and new classifications, it supports more accurate, personalised cancer treatment worldwide. It was developed in close collaboration with the American Joint Committee on Cancer (AJCC) and global experts.

The UICC TNM staging system is the common language in which oncology health professionals can communicate on the cancer extent for individual patients as a basis for decision making on treatment management and individual prognosis but can also be used, to inform and evaluate treatment guidelines, national cancer planning and research.

Arranged by anatomical region, this edition delivers updates to staging criteria for many tumour sites including, nasopharynx, HPV associated oropharynx, lung, thymus, mesothelioma, appendix, anal canal, cervix, vulva and prostate carcinoma. The 9th edition also reflects a growing emphasis on personalised care, incorporating refinements in prognosis aimed at aligning staging with patient outcomes.

Key changes in 9th edition

  • New staging for HPV-associated oropharyngeal cancers to better reflect their distinct behaviour and prognosis
  • Revised criteria for salivary gland and nasopharyngeal cancers, based on updated imaging and anatomical features
  • Anal canal tumours with larger T3 classifications now staged higher due to poorer outcomes
  • New classifications for medulloblastoma, cutaneous lymphoma, parathyroid carcinoma, adrenal medulla and extra-adrenal paraganglia tumours;
  • Revised and updated sections on Essential TNM and paediatric cancer staging;
  • Revised classification of prognostic factors for many malignancies.

These updates are grounded in recent evidence and reflect input from cancer registries, clinical outcomes, and international collaboration with the American Joint Committee on Cancer (AJCC) and disease-specific experts.

UICC recommends that the TNM Classification 9th edition take effect as of 1st of January 2026.

Investing in prevention measures and ensuring that they are included in universal health coverage (UHC) schemes is a cost-efficient and effective way to save lives and reduce the cost of cancer to public health systems. An upcoming UN high-level meeting provides an opportunity to prioritise cancer control as an essential part of achieving UHC.
